If you have recently completed a course with Star Training and Assessing and wish to purchase a physical plastic wallet card, please complete PLASTIC WALLET CARD PRINT FORM.

Please note: Star Training & Assessing can only issue cards for students completed their course with Star Training & Assessing and cannot issue or re-issue qualifications completed with other registered training organisations.
